8. Signature

By signing an application form, you promise that what you said is true and complete. You also indicate that you understand and accept all statements provided on the form. Never sign a form if you can’t accept all statements.

Writing the signature:

  • Make sure the signature matches the name you listed in the personal information. This generally is your first name, middle initial and last name.
  • Never use a nickname in your personal information or signature.
  • Make sure your signature is clear and legible.
